The University is managing the process for obtaining Tax-Free 190 proof and 200 proof Ethyl Alcohol. Procurement Services Strategic Contract Management is working in conjunction with Fisher Scientific to deliver Ethyl Alcohol.

Tax-Free Alcohol Procedures

Room Delivery

• Departments print and complete the “U-M Requisition for Tax-Free Alcohol Form.” Click here to download the form in a PDF file.
  
  
• Fax the completed and signed form to Fisher Scientific at 734-615-6877.
  
  
• There is a $21.50 HazMat fee for all total orders placed with Fisher so please consolidate orders.
    
• A signature is required for all alcohol deliveries.
  
  
• Departments pay restock, return shipping and hazmat fees for all refused or non deliverable orders.
